What is Actually Going on With Your Achilles?
The Achilles Tendon is the largest and strongest tendon in the body, connecting your calf muscles to your heel bone. It’s responsible for every push-off, plant, cut, and jump you make on the court and in your sport!
When you Develop Achilles Tendinopathy, the tendon undergoes degernative changes – NOT inflammation, despite what most people think. The tissue breaks down faster than it can recover, usually from repetitive stress without adequate loading progression.
Left unaddressed or improperly treated, that breakdown compounds. What starts as morning stiffness and mild heel pain after practice can progress into chronic tendinopathy, partial tears, or unfortunately full ruptures.
Why Most Achilles Injuries Get Worse
The window between a nagging Achilles and a Ruptured one is narrower than most athletes realize.
Here’s what happens when rehab is ignored, rushed, or done improperly:
- Tendon tissue weakens progressively. Degenerating tendon can progress to partial or complete rupture
- Pain-Guided erst without loading actually replays recovery. The Tendon NEEDS progressive mechanics stress to remodel properly
- Return to Sport based on pain reduction alone, NOT Functional testing, is the MOST common cause of re-injury
- Missing 1-6 months for Tendinopathy is common…Missing 12-16 months for a rupture is the reality no one plans for
THE TRUTH: REST is NOT REHAB. Passive rest without progressive loading doesn’t restore tendon capacity, it leads to further atrophy. Every week of inactivity without structured loading sets your return-to-sport timeline back. This is one of the most common and costly mistakes we see in clinical practice.
THE TRUTH: This is when unfortunately what leads to a partial or complete rupture of the Achilles Tendon. Tendons are notoriously poor at communicating severity in real time. You can play on a significantly degenerated tendon right up until you can’t. The absence of sharp pain doesn’t mean the tendon is healthy — it means it hasn’t hit its threshold yet.
THE TRUTH: Ice and stretching address symptoms, not the root cause. Tendinopathy is a capacity problem — the tendon can’t handle the load being placed on it. The only evidence-based solution is a graded loading program that progressively builds that capacity back up.
What Proper Achilles Rehab Looks Like:
It's not just calf raises!
Evidence-Based Achilles Tendon Rehab follows a specific tissue-loading progression.
Skip or rush phases, and you risk re-injury and set-backs.
Rush loading and the tendon responds with more breakdown.
Here’s The Basketball Doctors Rehab Process & Approach Looks Like:
Phase 1: Pain Management & Isometric Loading
Goal: Reduce Pain & Maintain baseline tendon stimulus
Isometric calf contractions provide tendon load without aggravating the tissue.
Phase 2: Isotonic Loading
Goal: Rebuild tendon capacity through slow eccentric and concentric calf work.
Heavy slow resistance drives tendon remodeling
Phase 3: Energy Storage & Return to Running
Goal: Restore the tendon's ability to store & release energy - critical for cutting, sprinting and jumping
Introduce plyometrics, change of direction drills, and progressive running
Phase 4: Sports-Specific Training & Return to Play
Goal: Full basketball activity - Court drills, Reactive movement, & Game simulation
Return-to-play is cleared based on objective criteria, not just symptom reduction
Phase 5: Performance Training & Maintenance
Goal: Develop long-lasting habits, strength, power, and movement mechanics to make sure this doesn't become a recurrent problem.

Frequently Asked Questions
It depends on severity, how long you’ve had it, and how well the rehab progression is followed. Mild to moderate tendinopathy typically responds well within 8-12 weeks of structured loading. Chronic cases may take up to 3-6 months. A ruptured Achilles Tendon requires 12-16 months. The most important variable is starting the right program early and staying consistent
In most cases and a short answer, yes — with modifications. We don’t pull athletes off the court unless absolutely necessary. Load management means adjusting how much provocative activity you do and when, while still keeping you in the game as much as possible. We’ll build your program around your schedule.
In the vast majority of cases, no. Research consistently shows that progressive loading-based rehabilitation produces outcomes comparable to surgery for chronic Achilles Tendinopathy – without the 12+ month surgical recovery window. Surgery is typically considered only after 6 or more months of structured, well-executed conservative rehab has failed. Ruptures are a separate discussion. If you are post-surgery, we will coordinate and communicate your post-operative rehab with your medical team.
Yes. Our online coaching program is build specifically for athletes who don’t have access to a basketball-specific or sport-specific DPT care locally. You get the same 1-on-1 program design, check-ins, and DPT oversight remotely. Athletes across the US and internationally have worked with us this way.
A self-guided program is static, it doesn’t know your injury severity, your training history, or what’s aggravating your tendon. We do. Every program we build is individually designed after an innate evaluation, and it evolves week to week based on your progress and your feedback. You’re not buying a program; you’re working with a expert and doctor of physical therapy.
Return to sport is cleared based on objective criteria, not just symptom resolution. We assess various muscles endurance and strength, hop testing, and sport-specific movement patterns before learning any athlete for full activity. For tendinopathy, that’s typically 8-16 weeks. For ruptures, 12-16 months. The goal isn’t just getting back on the court, it’s getting back on the court to full performance with a reduced re-injury risk.
